An alternative theory, the `anxiety or tension ; reduction' theory attributes causality to people with anxiety difficulties using substances, particularly alcohol to reduce their fear or worrying. Although there is a high comorbidity between alcohol use and anxiety, causality is not clear. Individual genetic make-up and differences in symptomatology and substance use i.e. amount, frequency, tolerance etc ; make it very difficult for a consistent finding of causality in research. Likewise, a common misconception is that cannabis use causes schizophrenia. This misconception has probably arisen due to the fact that schizophrenia tends to develop in late adolescence around the same time that cannabis use is peaking. However, as high numbers of people with schizophrenia also smoke tobacco, it would be just as incorrect to attribute development of schizophrenia to tobacco use. It is also notable that rates of schizophrenia have not increased while cannabis use has. Thus any connection between cannabis use and schizophrenia is likely to be a correlation rather than causal. In conclusion, from a practical point of view, the issue of primary diagnosis isn't that important unless picked up at early stages of the disorders. By later stages the two diagnoses are enmeshed and mutually reinforcing and azithromycin.
Despite the high probability that people with ankylosing spondylitis are positive for HLA-B27, a negative finding does not exclude this disease, as 5% of patients do not carry the antigen. And conversely, a positive HLA-B27 result does not confirm disease. More than 90% of carriers about 68% in the general populations of Europe and most parts of Asia will never be affected by ankylosing spondylitis. If there is simultaneous evidence of back pain of an inflammatory nature, however, the likelihood of ankylosing spondylitis is considerably increased. With respect to the risk assessment, a positive finding of HLA-B27 means that a thorough check must be made for associated symptoms. The very fact that an HLA-B27 test has been performed should give pause for thought, as leucocyte antigen is not tested routinely but only if inflammatory back pain or other symptoms give rise to the suspicion of ankylosing spondylitis or another HLA-B27associated disease. In cases of doubt, the underwriter should always ascertain the reason why this parameter was tested.

In 1995, the WHO Expert Committee on Onchocerciasis estimated that 17.7 million people were infected, of whom about 270, 000 were blind and another 500, 000 were severely visually impaired WHO 1995a ; . However, more recent information from rapid epidemiological mapping of onchocerciasis Noma and others 2002 ; by the African Programme for Onchocerciasis Control APOC ; indicates that the number of those infected is twice as high and that some 37 million people were infected in 1995. This revised estimate corresponds to an estimated 1.99 million DALYs lost because of onchocerciasis in 1995. Using the most recent rapid epidemiological mapping data and the latest APOC data on treatment coverage and assuming that four rounds of ivermectin treatment will reduce the prevalence of troublesome itching by 85 percent and the burden of visual impairment and blindness by 35 percent give a DALY estimate of 1.49 million DALYS lost for 2003 see table 22.1 ; . In addition to the burden of blindness and severe itching, onchocerciasis has important socioeconomic consequences. In the West African savanna, fear of blindness has resulted in the depopulation of fertile river valleys, severely affecting agricultural production. It was this socioeconomic impact, and not just the health impact, that led to the creation of the Onchocerciasis Control Program OCP ; in West Africa in 1975 Remme 2004b ; . Even though the importance of onchocercal blindness has long been recognized, only in 1995 did research demonstrate that the public health importance of onchocercal skin disease was even greater. Troublesome itching associated with dermal onchocerciasis makes working, studying, or interacting socially difficult Murdoch and others 2002; Vlassoff and others 2000 ; . Onchocercal itching now accounts for 60 percent of DALYs lost Remme 2004a ; . Other skin manifestations, such as reactive skin lesions, are not included in the DALY estimates, even though they are highly prevalent and have major psychosocial and economic impacts. Methods: 17 subjects, HIV + ; for 3 years on HAART and having undetectable plasma HIV-1 RNA viral load NASBA: 400 copies ml ; were evaluated mean CD4 count: 474.7 cells range 15-1100 SD 316.7 . RNA was extracted from a single rectosigmoid biopsy mean weight: 10mg, average yield 25 mg biopsy ; . HIV RNA was quantified using HIV LTR-specific primers by RT-PCR assay sensitivity 10 copies ; . Total RNA recovery is 95%. Results: 15 of 17 patients with undetectable plasma viral load 88% ; had detectable HIV in a single mucosal biopsy. Identical results occurred when a 2nd biopsy from the same site and same time was analyzed. Assay variation of the same extracted RNA run 120 days apart was 0.2 log; intra-subject variation between the 2 biopsies was 0.3 log. In biopsies from these 15 patients the mean number of copies of HIV-1 LTR per mg of total RNA was 215.8 range 44-768 SD 190.2 ; .
